I, hereby acknowledge that I am voluntarily participating in martial arts training and related activities at Nampon PK Muay Thai, and that such activities involve inherent risks of physical injury. I understand that these risks may include, but are not limited to, muscle strains, sprains, and broken bones, as well as more serious injuries such as permanent disability or death

I acknowledge that my participation in these activities is purely voluntary, and that I am participating in them of my own free will. I represent and warrant that I am in good physical condition and have no known medical condition that would prevent my participation in these activities.

I agree to assume full responsibility for any and all injuries or damage that I may sustain while participating in these activities.

I do hereby fully release and discharge Nampon PK Muay Thai, its owners, officers, employees, agents, and assigns, from any and all claims, demands, damages, actions, or causes of action, present or future, arising out of or connected in any way with my participation in these activities.

I acknowledge that I have read and understand this Waiver of Liability and Hold Harmless Agreement, and I am signing it voluntarily.
